Summary:

 

Under indirect supervision, provides, oversees, and/or administers a wide range of psychosocial evaluation/assessment, diagnostic, counseling therapy, crisis intervention services in a clinical or medical environment which requires a high degree of independent decision-making.  Most aspects of this job require clinical assessment, evaluation, and intervention skills to Substance Abuse and Mental Health consumers that are treated with individual, family, and/or group therapy in a medical setting.   Case management/supportive services for disposition of cases are also a function of this job.

 

 

 

Essential Duties and Responsibilities:

Provides clinical screening of potential Mental Health and Substance Abuse patients in the Primary Care setting using a combination of approved screening tools.  Utilizing clinical psychosocial assessment and treatment services to consumers with substance abuse and mental health problems.  Face to face clinical assessments are scheduled and provided using necessary clinical skills.   Provides evidenced based clinically effective treatment services for outcome measures.

Service delivery may also involve consultation with various community stakeholder agency staff, Primary care offices, Health Department, Network Coordinators, family members or support network as deemed necessary or clinically appropriate. Provides indirect services with a variety of stakeholders from whom services are received or needed by consumers to insure appropriate coordination for transition of services to or from another provider or agency.

Provides indirect services (e.g., case management and supportive services) with a variety of stakeholders from whom services are received or needed by consumers to insure appropriate coordination for transition of services to or from another provider or agency.

Administer and enter data entry into Electronic Health Record system for research and tracking of program requirements. Provide center based clinical assessments as needed. Other duties as assigned by the supervisor

Consults with other local providers and referral sources by telephone as needed to insure bi-directional linkages between primary care and mental health or substance abuse services.

 

 

 

Screened consumers Minimum Requirement:

 

The clinician is expected to collect the necessary information of any consumers appearing for the screening and assessment.  The clinician is also expected to submit appropriate paper/electronic/data entry work so that screening tools, assessments, intakes, evaluations, assertive outreach and case support may be submitted for documentation deadlines.  

 

 

 

Special Attributes:  Strong evaluative and clinical assessment skills needed. Must be able to get along well with other professionals to build collaboration in the work environment of multiple providers, community agencies, and a variety of community stakeholders. Must be able to engage consumers and provide evidenced based interventions with immediate impact on consumer’s behavior. Must be able to provide brief therapy with immediate impact on consumer’s behavior. Must have the ability to gain information, make assessments and build rapport with consumers and families, make sound decisions in emergency situations, and provide evidenced based clinical interventions. Must be able to provide written and oral reports to supervisor or stakeholders. Must possess strong organizational and computer skills required including various office software and the internet.

 

Supervisory Responsibilities:  None unless supervising a student and/or training a co-worker.

Education and/or Experience: Master’s degree in Social Work, Psychology, counseling, substance abuse, or marriage and family therapy from an accredited college or university and licensed eligible or provisionally licensed in their perspective field.  Applicants with master’s degrees who are provisionally licensed or licensed eligible as clinical social workers, professional counselors, substance abuse counselors and marriage and family counselors will be given consideration for employment.
